Notice of Privacy Practices

1. Our commitment

The clinicians who treat you through NuvoRx.net are covered entities under HIPAA, and NuvoRx.net acts as their business associate. We are required by law to maintain the privacy of protected health information (PHI), give you this notice, and follow its terms.

2. How we may use and disclose your PHI

Treatment: to provide, coordinate, and manage your care, including sharing information with pharmacies, laboratories, and referral clinicians.

Payment: to bill and collect for services, including verifying eligibility and processing payments.

Health care operations: quality review, clinician credentialing, training, and business management.

As required by law: public health reporting, health oversight, judicial orders, and law enforcement requests permitted by regulation.

3. Uses requiring your authorization

Most uses of psychotherapy notes, marketing communications, and any sale of PHI require your written authorization. You may revoke an authorization at any time in writing.

4. Your rights

You have the right to inspect and obtain an electronic copy of your records, request amendments, request an accounting of disclosures, request restrictions on certain uses, request confidential communications by alternative means, and receive a paper copy of this notice.

You have the right to be notified following a breach of unsecured PHI.

5. Complaints

You may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights, at hhs.gov/ocr/privacy/hipaa/complaints. You will not be retaliated against for filing a complaint.

6. Changes to this notice

We reserve the right to change this notice and to make the revised notice effective for information we already have as well as information we receive in the future. The current notice will always be posted on this page.
