Telehealth Informed Consent

Before receiving care through NuvoRx.net, patients review and accept this informed consent. It explains how telehealth works, what it can and cannot do, and your rights.

1. What telehealth is

Telehealth is the delivery of health care using secure video, audio, messaging, and electronic questionnaires instead of an in-person visit. Your clinician is licensed in the state where you are located at the time of the visit.

2. Benefits

Access to licensed clinicians without travel or waiting rooms, more flexible scheduling, and continuity of care through secure messaging between visits.

3. Risks and limitations

A clinician cannot perform a hands-on physical exam. Information transmitted may be insufficient for a diagnosis, requiring in-person evaluation or laboratory testing. Technology failures may delay or interrupt care, and — despite security safeguards — no electronic transmission is entirely free of risk.

Telehealth is not appropriate for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department. For a mental health crisis, call or text 988.

4. Prescriptions

A prescription is issued only when a clinician determines it is medically appropriate after evaluating you. No medication is guaranteed. Controlled substances are not prescribed through this platform. Prescriptions are sent to an independent, state-licensed pharmacy of record.

5. Your rights

You may withhold or withdraw consent at any time without affecting your right to future care. You may request an in-person referral, request copies of your records, and ask questions about your treatment at any point.

Visits are not recorded without your written permission. Your records are handled in accordance with our Notice of Privacy Practices.

6. Alternatives

Alternatives to telehealth include in-person evaluation with a local clinician. Your clinician will advise you if in-person care is more appropriate for your condition.
